If the unit runs out of battery will it run with the charging lead plugged in?

The unit runs off a rechargable lithium ion battery, similar to your phone and laptop. If you can physically make the distances (with cords etc) safely work, there should be no reason why you can't operate the unit while it is charging, as long as your power supply has sufficient amperage.
Will the unit still heat if using an aftermarket 4L/min pump or only with the 2L pump provided.?

No, the Companion Aquaheat NXT is not compatible with an aftermarket water pump. The unit is designed to work with its own specific pump and hose.
Does the pump turn off and on with the shower head ( pressure sensitive) or does it run continuously in standby.

Yes, the pump will run continuously whilst the unit is in operation but the water flow can be switched on/off from the shower head.
As you know when camping water is like gold if you have to take it yourself, How much wasted water is there until the unit starts pumping hot water out.?

The unit will only ignite once there's water moving through it, so it starts to heat up the water straight away but does build up to the set temperate over say 30 seconds.
This is common with pretty much all 12v camp showers though - usually you wouldnt just let the water run off and get wasted, you'd hold the shower head over the water recepticle (bucket, or jerry can) and recycle the water through until its up to temperature.
Can you connect a water hose directly instead of the pump?

The fittings on the new NXT are the same size as standard hose fittings, however the unit is NOT designed to be used with mains pressure.
The maximum flow rate is 2L p/min, so you would need to source a flow regulator yourself, and limit the mains water source to 2/l p/min flow before you consider connecting to the Aquaheat.
